Job Description

Responsible for providing comprehensive Patient Experience Manager services to patients in conjunction with clinical counselors and clinical team. 

Responsibilites

The duties listed below reflect the general details considered necessary to describe the principal functions of the job as identified and shall not be considered as a detailed description of all work requirements that may be inherent in the position. 

  • Ensure all Patient Experience Managers identified needs and documentation is up to date.   
  • Works with patients to develop individualized case management plans. 
  • Shares pertinent information with team as obtained from patient’s family, referral sources, or legal entities as indicated throughout the program. 
  • Facilitates group sessions.  
  • Continually monitor and evaluate each patient’s progress through their case management plan and develop corrective action revisions to the plan as needed. 
  • Maintain accurate documentation of service objective and outcomes as well as other services in accordance with Federal, State, and County guidelines. 
  • Assists patients in obtaining necessary resources to achieve case management goals. 
  • Actively participate in patient and program related meetings and trainings. 
  • Maintain patient related KIPU data, including case notes. 
  • Adheres to strict boundaries and professional ethics in the care of patients. 
  • Conducts crisis intervention as needed. 
  • Communicates and works effectively with a team, consults with the multi-disciplinary team, patient, and referent (consistent with CFR 42 and CFR 45) to address clinical issues both verbally and in writing related to the individualized treatment plan, continuing care, including types of care, level of care.  
  • Regular and predictable attendance is an essential function of the job.  
  • Performs other duties as assigned. 

This is a full-time position working Sunday-Thursday 8am-4pm

Qualifications
  • Master’s degree or higher, in chemical dependency or related field, with a practicum in health or human services agency, preferably within a drug and alcohol setting OR 
  • Bachelor's Degree with a major in chemical dependency or related field, with a one (1) year of clinical experience in health or human services agency, preferably in a drug and alcohol setting OR 
  • Associate Degree with major in chemical dependency or related field and two (2) years of clinical experience in health or human services agency, preferably in a drug and alcohol setting. 
  • Combination of education or business experience. 
  • Knowledge of addiction/recovery processes along with related resources.
