What We’re Looking For:
The Population Health Nurse (PHN) is a member of the Population Health unit. The Population Health Nurse’s role will comprise of diverse job functions and program assignments designed to meet specific contractual and program related requirements which includes an approach that treats the population as a whole. Although, the PHN is a direct report under Population Health Management, the PHN works in a matrixed relationship and is accountable for meeting the goals, criteria, and business protocols of the with other supporting departments served. The PHN will address the promotion of health and wellness and the prevention of disease. This position will utilize population health management approaches to care in an effort to move beyond episodic patient encounters to address more longitudinal health outcomes. The PHN will contribute to the attainment of specific healthcare metrics with the aid of identifying individualized care needs through health record and systems reviews, member interviews and assessments, community coordination and the inclusion of other resources as needed. The main focus will include gathering and analyzing member-centric and population-level data, promoting wellness and disease prevention within identified populations, adopting and disseminating best practices for population health, and identifying specific populations who may benefit from greater outreach efforts to promote health screening and related primary care services. The health outcomes that are derived from facilitating prevention and treatment methods are then researched and combined with the existing data, which will result in support of the individual or community amendments to refine prevention and treatment techniques. This position is responsible for providing intensive nursing care management and/or collaborative consultation to support individuals in identified Special Needs Populations that are high risk or high cost.
On a typical day, you might:
- Provide intensive care management/care coordination and follow up.
- Ability to identify trends in the underutilization or overutilization of services.
- Collaborates with Trillium clinicians or other members of the Specialty Unit.
- Analyze population-level data from electronic health records and other sources/platforms available.
- Undertakes population health/integrated care initiatives.

Qualifications:
Education: RN
Required Experience:
Minimum of two (2) years of experience working with the MH/SU/IDD population and at least three (3) years as a Registered Nurse.
Preferred Experience:
Knowledge of QM, UM procedures as well as experience in using data analytics for population health management preferred.
License/Certification:
Fully licensed by the North Carolina State Board of Nursing required.
Preferred: Certified Managed Care Nurse Certification or certification in care management preferred.
Must have a valid driver’s license
Must reside in North Carolina to be considered for remote status
