Job description

Job Title: Nurse Practitioner / Physician (Connecticut Licensed)

Job Overview

Medigence Health is seeking a dedicated Physician or Nurse Practitioner to join our team in conducting Health Risk Assessments. This remote position allows for flexibility while making a significant impact on the lives of Medicare, Medicaid, and Commercial Marketplace members.

Key Responsibilities

  • Conduct comprehensive evaluations of medical and social assessments for beneficiary members.
  • Identify and refer patients for routine, urgent, and emergent case management as necessary.
  • Provide recommendations for patients to follow up with their primary care physicians.
  • Travel throughout Connecticut, and potentially neighboring regions, where you hold a valid license.
  • Execute assessments in various environments including patient homes, clinics, and via telehealth.
  • Accurately document evaluations using the available Electronic Medical Records (EMR) system.

Required Skills

  • Minimum of 2 years of hands-on experience as a Nurse Practitioner or Physician.
  • Proficient knowledge of HCC model, HEDIS, and STAR quality measures.
  • Must possess a current, unrestricted license in Connecticut.
  • Registration with Medicare is essential.
  • Board certification in internal medicine, geriatric, or family medicine specialty is preferred.
  • Strong communication skills, particularly with elderly and chronically ill patients.
  • Comfortable utilizing telehealth technology and proficient in computer, tablet, and mobile device usage.

Qualifications

  • Current unrestricted license in the state of Connecticut is required.
  • Residence in Connecticut is mandatory.
  • This role does not require the prescribing or alteration of existing treatment plans for members.
