Job Description:

The Medication Adherence Specialist supports patients in managing their prescribed medication regimens, ensuring adherence to treatment plans. The specialist collaborates with healthcare providers, pharmacists, and patients to identify barriers and develop strategies to enhance medication adherence.

Essential Functions:

  • Evaluate patients' medication regimens and identify potential adherence barriers.
  • Create and implement tailored adherence strategies using reminders and tools.
  • Conduct regular follow-ups to track adherence and adjust plans as needed.
  • Work with physicians and pharmacists to ensure coordinated care and communication.
  • Recognize and assist in overcoming barriers such as financial issues and side effects.
  • Maintain accurate records, analyze data, and report on adherence metrics.
  • Ensure patients receive necessary support and access to resources.
  • Keep current with best practices and train other healthcare staff on adherence strategies.
  • Possess relevant education and experience and exhibit strong communication and organizational skills.
  • Other duties as assigned

Knowledge, Skills and Abilities:

  • In-depth understanding of medication regimens, pharmacology, and potential side effects.
  • Awareness of common barriers to medication adherence and strategies to overcome them.
  • Familiarity with healthcare systems, insurance coverage, and patient assistance programs.
  • Understanding of a pharmacy workflow including filling patterns, copay billing and pharmacy discount programs.
  • Proficiency in using electronic health records (EHR) and other healthcare technologies.
  • Excellent organizational and time-management skills for handling multiple tasks and follow-ups.
  • Analytical skills for monitoring adherence data and identifying trends.
  • Ability to develop and implement personalized adherence plans.
  • Capability to work collaboratively with healthcare providers and coordinate patient care.
  • Aptitude for advocating on behalf of patients to secure necessary resources and support.
  • Capacity to stay current with the latest research and best practices in medication adherence.
  • Competence in maintaining patient confidentiality and managing sensitive information.
  • Proficient in Microsoft Office Suite

Qualifications:

  • High School diploma/GED, advanced degrees or certifications in relevant fields (e.g., Certified Pharmacy Technician, Certified Health Education Specialist) are preferred but not required.
  • 1-3 years’ experience as a Pharmacy Technician, Medical Assistant, Medical Records Technician, or Referral Coordinator
  • 1-3 years’ experience working with medical records and/or patients in a healthcare setting
  • Home office that is HIPAA compliant for all remote or telecommuting positions as outlined by the company policies and procedures
  • Proficiency with electronic health records (EHR) and other healthcare technologies.

Salary Range:

$16.47 - $22.00 hourly
