POSITION SUMMARY:
Holon Health is seeking a compassionate and highly organized Case Manager to jour our collaborative care team. The Case Manager will play a crucial role in proactively monitoring client progress, ensuring adherence to treatment plans, and providing ongoing support to facilitate positive treatment outcomes. This position offers a unique opportunity to make a significant impact on the lives of individuals in recovery and contribute to a patient-centered, integrated care model.
SCHEDULE & LOCATION
This is a full remote role, but the individual must be physically located in the United States. Hours will typically be 40 per week with some flexibility around when those hours occur.
