Job description

Overview:

We are seeking a dedicated and skilled Clinician to join our primary care team, focusing on integrating behavioral health services. This role involves collaborating with third-party organizations and behavioral health hubs to enhance patient care quality. The ideal candidate, a nurse or social worker nearing completion of their PMP, should possess strong project management and data analytics skills, enabling them to lead meetings and co-facilitate discussions while setting clear agendas. Essential functions include conducting patient assessments, developing treatment plans, coordinating care across disciplines, and contributing to quality improvement initiatives.

Qualifications:

  • Bachelor’s or Master’s degree in Nursing or Social Work
  • Active clinical licensure (RN or LCSW)
  • Almost completed Project Management Professional (PMP) certification
  • Experience in data analytics and project management
  • Strong communication and leadership skills

Essential Functions:
