This position has the ability to work remotely on either a part- or full-time basis as determined by MMG leadership. Candidate must be located in Michigan.
Position Overview: Responsible for gathering and coordinating patient data and disease management analytics to understand health trends, determinants and access impediments for select populations of patients. Using data analyses, recommends and participates in the implementation of initiatives, population-oriented collaborations and other efforts to improve the health of the selected populations. Provides project coordination support as assigned by direct leader to meet department and organizational goals.
Essential Functions and Responsibilities:
Responsible for collecting and analyzing data used to support Population Health initiatives.
Develops reports to assist department leaders and committees in process improvement initiatives.
Uses department specific software programs for capturing and reporting revenue, utilization rates and patient outcome opportunities.
Assists in new workflow development and implementation, identifying opportunities for improvement, research and resolution, including development of procedures and staff training.
Prepares Population Health-based presentation materials and attends meetings as assigned; responsible for providing updates to direct leadership and follow up on open issues.
Coordinates assigned projects for the department to include establishing timelines, meeting deadlines, coordinating internal and external resources.
Responsible for keeping the organization informed of project milestones, resources or time constraints, etc. that impact each project.
Develops and implements processes to monitor payor contractual obligations and ensure MPP’s compliance with contract obligations.
Works collaboratively with all departments to support the MPP’s initiatives and assist with research and resolution of issues.
Other duties as assigned or when necessary to maintain efficient operations of the department and the Company as a whole.
Required:
Associate degree in business, information services or related field; or complete of a certification program such as APM (Associate in Project Management), CAPM (Certified Associate in Project Management).
Two (2) years’ experience in process or project management.
Preferred:
Bachelor’s degree in business, information services or related field.
Two (2) years’ experience and knowledge of Population Health organizations, Managed Care, HMO or PPO operations.
Knowledge of population health revenue cycle processes.
