Job Title & Role Description:

The Supervisor, Nurse Care Manager plays a pivotal role in overseeing the day-to-day activities of the Nurse Care Managers, ensuring smooth and efficient delivery of patient care across the organization. This role focuses on supervision, training, performance management, and continuous education while ensuring optimal care coordination. The Supervisor will work closely with the Clinical Operations team to ensure the seamless management of patient care, particularly around transitions of care, discharge assessments, and the implementation of care plans. The Supervisor is responsible for maintaining the consistency of workflows and ensuring that the Nurse Care Managers are equipped to provide high-quality, patient-centered care. This role also involves contributing to strategic decision-making, mentoring team members, and supporting the operational aspects of care delivery.

Skills Required:

  • Unrestricted registered nursing license in the state(s) of care management activities
  • At least 5 years of experience in a health-related field, supporting patient care delivery, public health, or population health initiatives
  • Demonstrated expertise in care management and care coordination, particularly for complex patients with multiple healthcare needs
  • Experience with EHR or CRM systems for real-time documentation
  • Strong organizational skills, with the ability to prioritize and meet deadlines efficiently
  • Experience with supervisory responsibilities, including performance management, coaching, and training
  • Proficiency in Microsoft Excel for data analysis and reporting
  • Excellent oral and written communication skills, capable of conveying information clearly and effectively
  • Ability to maintain professional boundaries with both team members and patients
  • Cultural humility and the ability to engage with diverse community members
  • Technologically savvy, with experience utilizing systems such as PowerBI (preferred)
  • Ability to work remotely full-time, with a flexible, adaptable mindset
