Clinical Pharmacist

Santa Monica, CA

Responsible for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Follow department, facility, and regional policies, procedures, and guidelines related to medication use system, pharmaceutical care, and documentation of clinical interventions. Process medication orders in an accurate and timely manner according to established procedures. Review patient records and orders to assess patient pharmaceutical needs based on disease specific characteristics, co-morbidities, and clinical status.

Employee has option to telecommute from any state where Providence has offices (AK, WA, OR, CA, MT, NM, TX).

Pay Range: $117,645 - $236,600

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
