The OT will work with a dedicated team of homecare clinicians of RN's, PT's, Medical Social Workers, LNA's and a long term Manager.  Staff are proud to say "I work for Home Health & Hospice Care!"  Consider the possibilities!

Responsibilities:

  • Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain.
  • Evaluates results of occupational therapy by evaluating by observing, noting, and evaluating patient’s progress; recommends and implements adjustments as needed.
  • Determines occupational therapy treatment plans in consultation with physicians and homecare team.
  • Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs; recommending and providing assistive equipment.
  • Documents patient services in measurable goals.

Qualifications:

  • Knowledge of the principles, concepts and methodologies required of Occupational Therapy and it's practical application in the treatment of patients at home
  • At least one year of experience in the hospital setting, facility or orthopedic center. Homecare experience preferred
  • Bachelor's degree and NH Licensure as an Occupational Therapist
  • CPR certification
  • Valid drivers license, reliable vehicle and auto insurance
  • Demonstrated problem solving, empathetic communication and time management skills
  • EPIC (EMR) experience a plus! 

 Required:

  • Registered as Occupational Therapist with New Hampshire’s Board of Nursing.
  • CPR Certification.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.
