The Role:

We are looking for a dedicated and experienced Complex Case Manager (CCM) to join our team in delivering exceptional long-term mental health care to high-acuity clients, including those who are actively suicidal. The CCM will be instrumental in providing expert case management and support services focused on crisis stabilization and acute symptom reduction. The ideal candidate possesses a deep level of emotional intelligence, is skilled in navigating high-stress situations with empathy and independence, and demonstrates a proactive approach to client care. The Complex Case Manager will play a crucial role in nurturing a culture of trust, advocacy, and growth among patients, their support networks, and the Cerebral care team.

Who you are:

  • Bachelor's Degree in Nursing is required, with a preference for candidates holding a BSN and a Master's in social work, psychology, counseling, or a related field.
  • At least 2 years of psychiatric RN experience, including crisis management, risk assessment in the telehealth environment, and experience in managing clients with chronic or severe mental health conditions
  • An active RN Compact License is essential, with CCM certification preferred.
  • Skills
    • Candidates must possess excellent communication, organizational, and technological skills, and have experience with EMRs, Google Suite, Slack, Doxy.me, and RingCentral. 
    • You will cultivate a culture of trust, advocacy, and growth with patients, their support network, and our care team.
    • Comfortable with navigating nuance and pioneering new programs or initiatives.
    • Ability to organize and prioritize tasks independently.
    • Self-driven, adaptable, and solution-oriented.

  • Crisis Management
    • Evaluate potential crises identified by our Crisis Message Detection (CMD) system and follow the crisis response protocol when necessary.
  • Case Management
    • Manage a dynamic caseload of high acuity clients.
    • Conduct ongoing assessments, care planning, skill-building, goal management, and psycho-education.
    • Offer immediate crisis intervention for CCM clients in crisis situations; coordinate with emergency contacts and necessary resources.
    • Take charge of researching, identifying, and negotiating community resources to help clients tackle social determinants of health.
  • Clinical Quality 
    • Spearhead care coordination efforts with a client's Cerebral care team, family, and other social supports, along with healthcare providers as needed.
    • Facilitate ongoing clinical quality improvement projects.

The national base salary range (OR the national hourly range for nonexempt positions) offered for this position is outlined below. Cerebral is committed to equal pay for equal work; however, business reasons may dictate variations in pay that are attributed to objective factors, such as a candidate's qualifications and years of experience. Total compensation includes base salary and may include an annual performance bonus and equity/stock options.

National Base Salary Range:
$54,000$60,000 USD
