POSITION SUMMARY:
The Population Health Project Manager will be responsible for leading and managing population health and quality improvement initiatives aimed at enhancing healthcare services, patient outcomes, and overall organizational performance within the Department of Family Medicine (DFM) at Boston Medical Center. This role will ensure that the DFM patient population receives optimal management for both preventive care and chronic disease management. The Population Health Project Manager will work closely with cross-functional teams, and central VBC team at BMC to implement evidence-based practices, support Value-Based Care programs, and drive continuous improvement across the DFM department and the organization.
The Pop Health Project Manager will collaborate closely with the supervising manager and respective DFM leadership teams to develop strategies that ensure the success of the project and compliance with all contractual obligations. Additionally, the Project Manager will engage with analysts and EHR specialists to support reporting needs for the DFM department as whole, and or individual practice needs. They will also communicate the findings from these reports to the relevant stakeholders to individual practices.

EDUCATION:
Bachelor’s degree in health care or related fields. MS and/ or MPH preferred.

EXPERIENCE:
3-5 years of experience in healthcare settings or related setting is required, 1-3 years of experience in a Project Management role, and or attainment of a PMP certificate are highly preferred.
