Job description

POSITION SUMMARY:

The Population Health Project Manager will be responsible for leading and managing population health and quality improvement initiatives aimed at enhancing healthcare services, patient outcomes, and overall organizational performance within the Department of Family Medicine (DFM) at Boston Medical Center. This role will ensure that the DFM patient population receives optimal management for both preventive care and chronic disease management. The Population Health Project Manager will work closely with cross-functional teams, and central VBC team at BMC to implement evidence-based practices, support Value-Based Care programs, and drive continuous improvement across the DFM department and the organization.

The Pop Health Project Manager will collaborate closely with the supervising manager and respective DFM leadership teams to develop strategies that ensure the success of the project and compliance with all contractual obligations. Additionally, the Project Manager will engage with analysts and EHR specialists to support reporting needs for the DFM department as whole, and or individual practice needs. They will also communicate the findings from these reports to the relevant stakeholders to individual practices.

ESSENTIAL RESPONSIBILITIES / DUTIES:

  • Monitor and Evaluate Performance: Tracks and assess key performance indicators (KPIs), identifying opportunities for process improvements and enhanced outcomes.
  • Lead Quality Improvement Projects: Oversee the execution of quality improvement initiatives from start to finish, ensuring alignment with timelines, goals, and contract requirements.
  • Data Collection and Analysis: Collect, analyze, and interpret clinical and patient outcome data, as well as other relevant quality metrics, to inform and guide population health management programs.
  • Reporting and Communication: Produce and present regular reports that detail progress, trends, and areas needing attention to respective leadership teams and staff.
  • Dashboard Utilization: Develop and utilize dashboards to track and measure the effectiveness of ongoing programs and pilot projects.
  • Continuous Improvement Analysis: Conduct detailed analyses of collected data and provide actionable recommendations for ongoing improvement initiatives.
  • Chart Abstraction: Abstract clinical data for required VBC contract reporting and other quality incentive programs to ensure high-quality outcomes.
  • Foster a continuous Improvement Culture: Promotes an understanding of quality goals throughout the organization, fostering a culture focused on continuous improvement
  • Pilot Projects and Program Scaling: Collaborates with leaderships team to develop, implement and scale pilot projects aimed at improving quality care within the DFM department.

JOB REQUIREMENTS

EDUCATION:

Bachelor’s degree in health care or related fields. MS and/ or MPH preferred.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

None

EXPERIENCE:

3-5 years of experience in healthcare settings or related setting is required, 1-3 years of experience in a Project Management role, and or attainment of a PMP certificate are highly preferred.

KNOWLEDGE, SKILLS & ABILITIES (KSA):

  • Demonstrates a strong understanding of Value Based Care contracts and requirement and specifications within healthcare settings.
  • Possesses a solid grasp of quality improvement concepts including compliance and performance metrics.
  • Ability to thrive in a fast-paced environment, with strong organizational, critical thinking, and problem-solving abilities.
  • Strong analytical skills, including proficiency in Microsoft Excel for data analysis and reporting.
  • Proven ability to apply critical thinking and problem-solving techniques to address complex challenges in healthcare settings.
  • Ability to work effectively with diverse populations and navigate the unique needs of various patient groups.
  • Experience with EPIC/OCHIN and proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook.
  • Expertise in using computer systems to access, input, and analyze data efficiently.
  • Excellent verbal, written and interpersonal communication skills
  • Capable of summarizing data and outreach efforts to generate clear, concise reports for internal and external stakeholders
