Responsibilities

Conduct professional outreach calls to healthcare partners including hospitals, clinics, and treatment centers
Build and maintain relationships with social workers, case managers, and therapists
Share timely updates about provider availability and service openings with referral partners
Maintain detailed records of all healthcare partner communications and follow-ups including answering calls, scheduling patients and checking insurance eligibility.
Create and nurture ongoing relationships with key healthcare facilities
Provide comprehensive information about available mental health services and assisted living options
Track and document all outreach activities and relationship development progress
Coordinate with internal team members to ensure accurate availability information
