🎯 What You’ll Do in This Role:


Home Care, RN Case Manager, where you'll play a vital role in assessing patient needs, creating personalized care plans, and coordinating with a multidisciplinary team to deliver high-quality skilled nursing rights in the comfort of patients’ homes. This position calls for a clinically strong, compassionate nurse with excellent communication and organizational skills. Compensation is offered on a paid-per-visit basis.

Primary Responsibilities:

🩺 Patient Assessment

  • Perform comprehensive assessments addressing physical, psychological, and social aspects of each patient’s health.
  • Evaluate home environments to identify safety risks and barriers to effective care delivery.

📝 Care Planning

  • Collaborate with patients, families, and care team members to develop individualized care plans.
  • Establish realistic, patient-centered goals that reflect both clinical needs and personal preferences.

🔄 Care Coordination

  • Coordinate nursing care, therapies, and other medical services to ensure patients receive seamless, holistic care.
  • Maintain effective communication with physicians, therapists, and social workers to support continuity and quality of care.

📚 Patient Education

  • Educate patients and caregivers on diagnoses, medications, treatment plans, and self-care strategies.
  • Provide ongoing guidance for managing symptoms, preventing complications, and improving overall wellness.

📊 Monitoring and Evaluation

  • Continuously monitor patient progress and adjust care plans to reflect changing needs.
  • Track vital signs, medication compliance, and overall health status during home visits.

🗂️ Documentation and Reporting

  • Accurately document assessments, care plans, progress notes, and communications in compliance with regulatory and agency standards.
  • Record interventions and outcomes, including any changes in patient condition.

🤝 Collaboration and Advocacy

  • Work closely with interdisciplinary teams to support complex patient needs and drive positive outcomes.
  • Advocate for patients’ rights, preferences, and access to appropriate healthcare services.

📈 Quality Improvement

  • Engage in performance improvement initiatives to advance care delivery and maintain regulatory compliance.
  • Identify opportunities for improvement and apply evidence-based practices to enhance safety and patient satisfaction.
