Description
Hennepin Health is seeking a Community Outreach Coordinator to support our organization's community engagement initiatives, which includes building relationships with community organizations, potential members, and other county departments. This position links our health plan services and innovative care delivery model with the community stakeholders we depend on for our success. This person will represent Hennepin Health within the community and execute a strategy to maximize outreach presence within the Hennepin County region where we operate. The position will be part of Hennepin Health's Marketing, Communication, and Outreach team. However, they will also work closely with staff throughout Hennepin Health and Hennepin County, to ensure that community voices are considered in health plan operations.
Hennepin Health, a Hennepin County owned health insurance plan, provides health care for some of our most vulnerable residents. Hennepin Health's innovative practices have gained national attention and are considered a model for health reform.
Location and hours:
This position is hybrid and will be performed both on-site at the Health Services Building, 525 Portland Avenue South, 8th floor, Minneapolis, MN 55415, in the field and community-based settings, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. with occasional and weekend hours for community outreach events. While this position is designated as hybrid, based on current requirements, hires must reside in or relocate to Minnesota or Wisconsin.

In This Position, You Will
Implement a community engagement and outreach strategy for Hennepin Health.
Create and maintain strong relationships with community organizations and with Hennepin County departments serving Hennepin Health enrollees.
Represent Hennepin Health within the community and at events.
Collect feedback and outreach analytics that can be shared with the Marketing, Communication, and Outreach team and other Hennepin Health staff.
Support logistics for outreach initiatives such as scheduling, materials needed, and transportation.
Educate the public about Hennepin Health's three health insurance products, as well as its broader health reform vision and goals.
Need To Have
One of the following:
Bachelor's degree or higher in public health, nursing, behavioral/social science, or an approved related field.
Four years or more of related professional experience performing community outreach or health education activities.
Nice To Have
Bilingual in Spanish, Somali and/or Hmong is preferred, but not required.
A passion for working with the community and a desire to improve the health care delivery system.
Effective self-direction, work ethic, and intellectual curiosity.
Knowledge of:
Minnesota Public Health Care programs.
The health system, health plan operations, and county programs.
Experience:
Performing customer service and/or community outreach.
Working with a diverse client or customer population.
Candidates with fluency in another language
