What You’ll Do:

• Provide one-on-one telehealth coaching to assess patients’ dietary needs and health goals.
• Educate patients on meal planning, healthy eating habits, and weight management strategies.
• Serve as an advocate for patients’ nutritional needs by actively listening, addressing concerns, and collaborating with their healthcare team.
• Develop personalized nutrition care plans and monitor progress over time.
• Communicate professionally with care teams to ensure a seamless patient experience.
• Maintain accurate documentation of all patient interactions.

• A dedicated, empathetic professional with strong active listening and communication skills.
• Ability to work both independently and as part of a collaborative healthcare team.
• Bilingual (Spanish/English) candidates are a plus!
• Comfortable adapting to new processes and technology-driven healthcare solutions.
• Medical specialties: gastroenterology, geriatrics, home health, internal medicine, primary care, and urology.

• 2+ years of experience in clinical nutritional counseling, or dietetics care coordination.
• Experience in adult in-home, in-facility, or remote chronic conditions care management addressing nutritional needs.
• Ability to translate medical information, instructions, and documents to ensure comprehensive understanding.
• Demonstrated knowledge of nutrition education, coaching, and fundamental dietary principles.
• Previous experience in weight management.
• 2+ years of computer skills and EMR experience.

• 3+ years of experience in healthcare nutrition, chronic care management, and patient education.
• Degree in Dietetics or Nutrition preferred.
• Certifications:
  • Registered Dietetic Technician, or
  • Certified Nutritionist, or
  • Certified Health Coach

• Competitive hourly rate: $19.00 - $23.00 per hour, based on experience.
