The Opportunity  

 

We are looking for an experienced Registered Nurses to join our Clinical Programs team as a Care Coordinator to support the RITH program that Amplar Health delivers to patients in their home.

In this phone based role, you will be trained in all aspects of the RITH program so you can support particpants receiving care and the nurses delivering care in the community. 

Using your clinical skills, you will ensure a comprehensive patient centred management plan is achieved by:

 

  • Conducting initial phone assessment, care planning and goal setting.
  • Coordination of in-home services to support the rehabilitation and recovery of patients.
  • Monitoring patient progress and updating care plans as appropriate.
  • Building and maintaining effective relationships with a variety of different stakeholders including primary and secondary care organisations and your non-clinical support team.
  • Liaison with healthcare professionals and specialists.
  • Provision of telephone support, education, and advice.

 

This is a full time permanent position working predominantly Monday to Friday (between the hours of 8.30 -5.00 pm WA time) with the occassional shift on the weekend. This is a fully remote role i.e. work from home, however we are looking for candidates based in WA and who have a strong understanding of the WA healthcare system and services.

 

About You

 

As an outstanding communicator, you are comfortable liasing people on the phone (inbound and outbound) and your intuitive people skills will be central to your success. Ideally you have worked with post op patients and can support patients and their nurses with any post surgical complications.  You will be naturally empathetic, observant, attentive, trustworthy, and committed to making your patient feel safe and comfortable. You will be client and goal focused who is driven to achieve positive customer outcomes. You will also have:

 

  • Minimum 2 years' experience as a Registered Nurse with unrestricted AHPRA registration working in an acute setting.
  • Experience in care coordination or phone based role would be preferable.
  • Strong customer service orientation and relationship management.
  • Excellent clinical reasoning skills.
  • Ability to work in a fast-paced environment.
  • Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
  • Have a reliable internet connection speed.
