Job description

Welby Health is an exciting and growing San Diego-based company specializing in helping patients manage chronic diseases through technology and a clinical service model. Our vision is to make the world healthier by helping patients access the care and education they need to improve their lives.

We are expanding our clinical operations team to support our growing client and patient base. We are currently looking for FT Medical Assistant to assist in remote physiological monitoring, development and oversight of patient care plans, and guidance and support to our patient population. This is a fully remote position, and you will be able to use our proprietary software platform to optimize engagement with your patients. If you love to provide patient care and are tired of spending all your time 'charting', this is the job for you.

Nurses will work from home. The primary responsibilities include:

  • Review patient vitals and care plans in our proprietary technology platform.
  • Support a team of RN case managers to assist with patient care plans to address clinical, social, and behavioral needs.
  • Respond to alerts in patient vitals to understand how to improve their outcomes.
  • Provide education and guidance to improve health through video, telephonic and asynchronous encounters.
  • Act as a liaison with our physician clients and their patients to facilitate access to care.
  • Coordinating care with physicians.
  • Assist in helping patients develop healthy habits to improve their health over time.

Applicants must have a valid Medical Assistant certification.

Additional languages are is a plus.

Prior case management experience is a plus.

Experience with motivational interviewing is a plus.

Must have excellent customer service skills, be comfortable working with new technology, and be able to work from home.
