Cost Center: 85/Plan Based Care Management

FLSA Status: Exempt

Posting Salary Range: $62,712 - $77,564

Office Location: Remote within North Carolina.

What We’re Looking For

Trillium Health Resources has a career opening for a Population Health Operations Coordinator to join Populations Health Operations Team. This positions primary responsibilities function in a consultative and facilitative role on the Clinical Support Team that includes staff who perform tasks as liaison between Trillium and other provider entities in the community and our hospitals.

On a typical day, you might:

  • Create policies and procedures that often have department-wide impact and provides critical feedback on field impact of policies and procedures to enhance their effectiveness.

 

  • Manage, monitor, review and analyze short- and medium- term operations and services provided for adherence to program goals and objectives utilizing performance outcome data and reports.

 

  • Provide direction on Behavioral Health and specialty program Care Management objectives and measures of effectiveness; focuses attention on departmental and unit goals; recommends program changes to clinical management.

 

  • Utilize databases to analyze the effectiveness of current processes and organizational components; recommend changes on the basis of these analyses; train and assist staff in use of the databases.

 

  • Engage identified members into the care delivery system and utilize data and information from multiple sources to address members’ needs.

 

  • This role involves program development, clinical oversight, and conducting systematic program reviews for quality assurance to assess effectiveness and ensure compliance with established clinical standards, regulatory requirements, and best practices—ultimately driving improved outcomes for program participants.

 

  • Provide consultative and facilitative engagement with the targeted population, to include those with the most complex cases or problems.

 

  • Serve as a primary point of contact and liaison for key partner organizations, facilitating culturally competent care and coordination with tribal communities and other stakeholders.

 

  • Provide guidance on complex care coordination to high-risk members who may have identified needs with behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.

Qualifications: 

Education/Experience: 
 A Master’s degree is required with a minimum of three (3) years post Master’s-level fully licensed LCSW, LCMHC, LPA, LMFT or RN providing supervisory experience working directly with complex individuals with BH conditions.

 

Preferred Experience: 
 Prefer a minimum of three (3) years’ experience with care coordination serving members of various backgrounds such as but not limited to TRICARE, the North Carolina National Guard's Integrated Behavioral Health System, the Army Reserve Department of Psychological Health, the United States Department of Veterans Affairs, the Division of Juvenile Justice, and related organizations.

 

Prefer at least one (1) year of clinical policy review, conducting audits, or quality improvement review experience.

License/Certification:

Fully Licensed in North Carolina with a LCSW, LCMHC, LPA, LMFT or RN.

Must have a valid driver’s license

Location: Remote within North Carolina.

Must reside in NC to be considered for remote status.
