NTT DATA  currently seeks a Intake Coordinator to join our team in remote in the US.

Role Responsibilities:

  • Manage incoming calls to the Care Management Intake Line
  • Receive and process inquiries for all Care Management programs from internal referrals, community providers, emergency departments, primary care providers and other agencies
  • Provide outstanding customer services to internal and external customers
  • Triage and process referrals into the appropriate CM programs, actively seek consultation from clinical supervisors when needed to escalate a referral and support the Notice of Action (NOA) process
  • Effectively manage the Community Supports services referral process including referrals from Medical Respite
  • Triage and de-escalate calls from members who may be in crisis
  • Run monthly reports from various databases to refer into CM programs
  • Manage the voicemail system and input the information obtained into a database system
  • Follow up with referring entity on status of referral and act as point of contact until member is connected
  • Coordinate meet-and-greet meetings as requested by providers with the goal to exchange information about our programs

Required Skills/Experience

  • Associate or bachelor's Degree in related field; or 4 years of relevant experience.
  • Minimum one year experience in Triage, Intake or Care Coordination
  • Minimum one year experience working with people with complex health needs

Preferences:

  • Bi-lingual language skills preferred
  • Working knowledge and use of Trauma Informed language and principles a plus
  • Working knowledge and use of Harm Reduction language and principles a plus
  • Working knowledge and use of Person-Centered language and principles a plus
  • Experience with de-escalation techniques
  • Telephonic Case Management experience a plus

Where required by law, NTT DATA provides a reasonable range of compensation for specific roles. The starting hourly range for this remote role is ($24 - $25/hourly ). This range reflects the minimum and maximum target compensation for the position across all US locations. Actual compensation will depend on several factors, including the candidate's actual work location, relevant experience, technical skills, and other qualifications.

This position is eligible for company benefits that will depend on the nature of the role offered.
