The Navigator serves as a primary point of contact for referring physicians, patients, and caregivers providing resources for, and assistance with, accessing clinical and supportive care services within Rome Health and the community, including but not limited to facilitating patient appointments, including surgical referrals, labs, diagnostic areas, radiation, oncology, and other specialties as appropriate.
The Navigator provides clinical nursing support during interventional procedures and biopsies as appropriate to patient type.
The Navigator provides support to the hereditary high risk genetics screening program with respect to clinical assessment, results disclosures, and navigating patients through changes to medical management according to NCCN guidelines.
The Navigator serves as a public and facility educator; assisting with increasing access to community and social support services, working with community groups to increase awareness on a variety of topics, including screenings and guidelines.
Navigation & Clinical Duties
Serves as clinical care coordinator throughout the diagnostic process and continuum of care for breast imaging patients within the Imaging Center.
Clinical management of patients preparing for image guided breast biopsy; includes coordination of care with ordering MD, ambulatory surgery
Together with radiologist, meets with patients to discuss recommendations post imaging. Calls reports and charts findings in appropriate system, PACS, and CCS.
Educates patients prior to breast biopsy, surgery and throughout the diagnostic continuum of care.
Coordinates and schedules image guided breast procedures in Ultrasound, MRI, Mammography and image guided needle localizations.
Clinical assessment of patients prior to breast biopsy, report changes to radiologist and ordering MD.
Takes verbal orders when appropriate.
Assesses patient's pre and post procedure and initiates higher level of care if necessary. Calls report & charts procedures in CCS.
Follows up on all image guided biopsy pathology, reports findings to ordering physician offices, coordinates addendums with radiologists, ensures continuity of care.
Charts all breast biopsy and procedures in Clinical Care Station
Continuously reviews nursing processes and workflows to ensure patient safety, timeliness and compliance within Medical Imaging
Oversees, directs the activities, and provides meaningful feedback and evaluation of MI RNs and Non Clinical Women's Imaging Navigator
Assist with department needs and nursing coverage as needed
Education/Experience
Graduate of an accredited school of nursing
Two years' experience in a healthcare setting, preferably with a focus on women's health and breast care.
