Job Title: Clinical Navigator

Job Summary:

MIBH currently seeks a Clinical Navigator who will work within an integrated practice unit (IPU) setting to determine appropriateness for participation in the clinical program at the Institute, and to support family programming. The primary role of the Clinical Navigator will be to complete extensive medical record reviews and conduct intake assessment interviews with all applicants to gather and create a summary of necessary clinical data to assist the team in determining appropriateness of candidates for the MIBH program. The Clinical Navigator will also coordinate the family program portion of the Intensive Outpatient Program (IOP) in collaboration with other members of the IPU team by developing programming and providing psychoeducation to further improve the overall treatment services provided at MIBH. The individual in this position must adhere to the State of Colorado Code of Ethics of their relevant discipline.

Key Responsibilities:

  • Review all program applications, self-report surveys, and relevant medical records
  • Conduct 40–60-minute assessment interviews with applicants
  • Outreach to the applicant’s current providers to gather additional clinical information, if necessary
  • Create a 2-3 page written clinical summary of relevant medical, psychological, and psychosocial findings to present to the weekly Clinical Decision Team (CDT) to determine eligibility and appropriateness for the program
  • Notify ineligible patients of alternative resources based on recommendations from the clinical decision team
  • Support the Clinical Coordinator with substance reduction plans before MIBH evaluation if needed
  • Provide a verbal synopsis of the written clinical summary during the evaluation component of the program to ensure the clinical team has a thorough understanding of the patient’s medical, social, and psychological health needs
  • Support the intake team at MIBH with input on clinically related inquiries
  • Support the ongoing development of the family portion of the program at MIBH
  • Create innovative curriculum, psychoeducation, and programming to enhance family support in treatment at MIBH
  • Enter appropriate documentation into EPIC in a timely and accurate manner
  • Develop and maintain good working relationships with partner programs

Work Location:

Remote – this role is eligible to work remotely, but the employee must be in the United States.

The Marcus Institute for Brain Health (MIBH) at the University of Colorado Anschutz Medical Campus is a clinical care and research institute for Traumatic Brain Injury (TBI) and Psychological Health (PH) conditions such as post-traumatic stress, depression, and anxiety. The primary focus of the MIBH is to care for former military personnel who have separated from active duty, with special consideration for those who were separated with “Other Than Honorable” or “Dishonorable” discharge status due to TBI or PH conditions. The MIBH offers intensive outpatient care with detailed diagnostic and treatment services, frequently in a three-week program that integrates conventional medicine with integrative medicine.

Qualifications:

Minimum Qualifications:

  • A bachelor’s degree in biological science, nursing, health, human services, psychology, counseling, social work, or a directly related field from an accredited institution
  • 2 years of direct experience working with individuals with Traumatic Brain Injury and co-occurring Psychological Health conditions*
  • 1 year of direct experience in neurological rehabilitation services, philosophy, and practice*
  • 2 years of direct experience with active-duty service members, Veterans, or first responders*
  • Experience may be obtained simultaneously.

Preferred Qualifications:

  • A master’s degree from an accredited institution as a registered nurse or in social work is preferred
  • Experience using EPIC
  • Professional experience working with, or training in, family systems

Anticipated Pay Range:

The starting hiring range for this position has been established as $36.78 to $46.79 per hour.

The above hiring range represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
